These Forms should not be used if you are using an OTP device to Clock In/Out (i.e. ISAS/CFC).
Nurse’s Note Submission

- Effective March 13th, 2023 your paycheck will be processed based on nurse’s notes summited and client signed timesheet.
- 2 options are available for document submission (Read Full Policy for detailed requirements)
- Electronic Charting via Kantime (Direct Deposit or Paper Check)
- Paper Charting with Scan/Photo Submission via Kantime (Direct Deposit or Paper Check)
- You are required to choose an option by March 10, 2023. Failure to respond with and option will result in you being classified as a paper charting clinician.
- The current narrative PDF nurse’s note submission found on our website will be discontinued on March 13th, 2023
- Late document submissions may result in a delay of payment, partial payment, or non-payment.
- MAR/TAR and Logs will remain on paper
These forms are not to be used if you are currently using a phone and OTP device to clock in/clock out your time (i.e. ISAS and CFC staff).
Electronic Bi-weekly Timesheet Submission
As of 09/15/2022 ANCS will not be accepting any paper timesheets. Please use this form if you currently use/submit a paper time sheet.
